The Division of Clinical Forensic Medicine has developed a number of forms for use by medical practitioners working in this field. These forms are available for use, with due acknowledgement of the VIFM, by forensic practitioners. Please note that these forms contain sections which may not be required for all cases. The decision as to which sections of each form is used depends upon the case type and clinical judgement of the practitioner.
